3. Over the phone through the Veterans Park District Grant Park Recreation Center located at 44 W. Golfview Drive, Northlake, with a credit card (Visa, MasterCard, or Discover).

4. Via fax by downloading the Veterans Park District Registration Form and faxing to (708) 492-1801.

5. Mail by downloading the Veterans Park District Registration Form and sending it to 44 W. Golfview Drive, Northlake IL 60164.

Requests for refunds must be made prior to the start of a program. No refunds will be allowed once the program begins.
